first ensure the update is really approve in this computer group and the status is "not installed".
Then look at C:\Windows\WindowsUpdate.log to see if any error show up.
What type of file do you have publish EXE or MSI ? Do you have customized 'IsInstalled' rules and/or 'IsInstalleble' rules ?
