I bet Patch Code is the problem. Verify the patch code. Install patch on client, scan registry. There will be a registry entry for it. This update is checking for only 2 conditions. If it finds both conditions on the client, the update is already Installed. If not, it shows it in windows update. Sorry about missing that.
